The role of Honor Care Limited’s home care coordinator is to assist our home carers and clients by professionally and efficiently assigning the rotas, via an electronic rostering system.
It will be your responsibility to assign the right carers to our clients and also make sure that our clients receive continuity of care and support. This is by ensuring that our clients have the same carers visit them - taking into consideration sickness and holiday cover.
You will be responsible to make that clients know about their personal care team.
You will be responsible for all activities, making sure there is effective communication between the carers and clients at all times.
In an event whereby any carer is absence, you will be responsible to professionally and efficiently cover any gaps of the rota using the appropriate carer and effectively and timely communicate any changes to the client.
You will be required to report directly to the manager and also provide support to the field care supervisors.
You will also be required to interview potential carers, make sure that all applicants’ paperwork is satisfactorily completed as per Honor Care Limited and statutory requirements. You will also be responsible to ensure that the paperwork is signed and countersigned, checking all ID and work permits as necessary as well as applying for DBS checks. You will equally make sure that electronic records of carers are updated.
What's more, you will be responsible to make home care assessment bookings for the field care supervisor who will then meet with potential new clients, and you may be needed at times to undertake care duties or cover on-call telephone.
